We retain patient health records for at least 10 years from the day after the last date in which services were provided, or longer where required for a lawful purpose. Other personal information is retained by law. Information is securely destroyed or permanently de-identified when it is no longer required.

You have the right to request access to the personal information we hold about you and to request correction of information you believe is inaccurate, incomplete or out of date. We may need to verify your identity before releasing information.
